Veterinary Release & Emergency Care Authorization
This authorization allows J.Harmony Pet Care to obtain emergency veterinary treatment for your pet if you cannot be reached.
1. Authorization to Seek Care
The owner authorizes J.Harmony Pet Care to seek veterinary care for the pet if the pet appears ill, injured, or in distress and the owner cannot be reached. Every reasonable effort will be made to contact the owner and the owner's emergency contact first.
2. Choice of Veterinarian
Care will be sought from the owner's regular veterinarian when possible. If that clinic is unavailable or the situation is urgent, the pet may be taken to the nearest available veterinary or emergency animal hospital.
3. Financial Responsibility
The owner is fully responsible for all veterinary costs, medications, transportation, and related expenses incurred on behalf of the pet, and agrees to reimburse J.Harmony Pet Care promptly for any amounts paid on the owner's behalf.
4. Transport
The owner authorizes J.Harmony Pet Care to transport the pet by vehicle to and from a veterinary facility as needed.
5. Release of Liability
The owner releases J.Harmony Pet Care from liability for any illness, injury, loss, or death of the pet occurring while in care, except in cases of gross negligence, and understands that reasonable care will be exercised at all times.
6. Agreement
By signing below, the owner confirms this authorization is given voluntarily and remains in effect for all current and future stays unless revoked in writing.
