Pet Care Service Agreement
This agreement is between J.Harmony Pet Care (McDonough, GA) and the pet owner named below for pet sitting, boarding, drop-in visits, and related care services.
1. Services
J.Harmony Pet Care agrees to provide the pet care services requested by the owner, which may include boarding, pet sitting, drop-in visits, feeding, potty breaks, supervised outdoor play, and photo/video updates. Services, dates, and rates are confirmed with the owner before care begins.
2. Reservations, Deposits & Payment
A deposit is required to reserve a booking and is applied toward the total. The remaining balance is due at drop-off or pick-up unless otherwise agreed in writing. Deposits are non-refundable for cancellations made less than 48 hours before the scheduled start of care.
3. Owner Responsibilities
The owner certifies that all information about the pet is accurate and complete, including feeding instructions, medications, medical conditions, and behavioral history. The owner agrees to supply sufficient food, medication, and supplies for the length of the stay, and to provide reliable emergency contact information.
4. Health & Vaccinations
The owner certifies the pet is in good health, free of fleas, ticks, and contagious illness, and is current on all required vaccinations (including rabies). J.Harmony Pet Care may refuse or end care for any pet that appears ill or contagious.
5. Behavior & Liability
The owner discloses any history of biting, aggression, escaping, or destructive behavior. The owner is responsible for any injury or property damage caused by their pet and agrees to hold J.Harmony Pet Care, its owner, and household harmless from claims arising from the pet's behavior, except in cases of gross negligence.
6. Photos & Updates
The owner grants permission for photos and videos of the pet to be taken and shared with the owner as updates, and, unless the owner objects in writing, to be used on J.Harmony Pet Care's website and social media.
7. Cancellation & Early Pick-Up
Cancellations should be made as early as possible. Fees for booked days are not refunded for early pick-up unless otherwise agreed.
8. Agreement
By signing below, the owner confirms they have read, understood, and agreed to the terms of this agreement, and that they are the legal owner of the pet(s) described.
